The NEET Application Form

The application process for NEET UG involves five steps:

  1. Registration
  2. Filling out the application form
  3. Uploading the scanned documents
  4. Payment of Application Fee
  5. Printing the acknowledgment receipt

A candidate should start the process by downloading the information brochure followed by registration. After filling the details in the application form, the candidate needs to upload his/her photograph and digital signature. And finally, make the payment for the application fee, and wait for confirmation.

Let us look into the details to be filled in the NEET UG application form.

Step 1: Registration

To complete the registration process, the candidate will have to enter the following details in the application form:

  • Candidate’s Name
  • Mother’s Name
  • Father’s Name
  • Date of Birth
  • Gender
  • Nationality
  • State of Eligibility
  • Category
  • Person with Disabilities
  • Identification Type
  • Identification Number
  • Mobile Number
  • Email ID

Candidates will have to verify the information prior to submission.

  • Candidates will have to create their password, retype the same and enter a security question and choose the desired answer.
  • Candidates will have to click on the OTP verification and verify it using the OTP sent to the registered mobile number.
  • The candidate will be able to see the Provisional Application Number for NEET. Make sure to note it down as it will be the login ID for all future logins.

Step 2: Filling out the application form

In the second step, the candidate will have to click on the link provided to continue to fill the NEET application form. Please make sure to fill the details in the form as per the class 10 certificate. Candidates will have to provide the details mentioned below:

Personal details (from registration)

Candidate’s name

Mother’s and father’s name

Gender

Category

Date of birth

Nationality

Identification type

Identification number

Candidates will also have to answer the questions:

Are you a person with disability?

Do you belong to the minority category?

Are you suffering from diabetes type 1?

Place of birth

Address of birth

State of birth

District of birth

Choice of medium for

the question paper

The candidate is supposed to select the medium of the question paper from the languages English, Hindi, Gujarati, Marathi,  Bengali, Oriya, Assamese, Telugu, Tamil, Kannada, and Urdu
Academic details (class X or equivalent)

Year of passing

Percentage of marks obtained

Address The candidate needs to provide the permanent as well as correspondence address at the time of application.

Details of parents/

Guardians

Details such as the qualification, occupation, and annual income of the parents/guardians are required.
Dress code details Candidates must indicate whether they intend to wear customary dress adhering to the dress code while appearing for the exam.

Step 3: Uploading the scanned documents

In this step, candidates will have to upload the scanned copies of their photograph and signature as per predefined specifications. Please note, candidates should write their name on the photograph along with date otherwise application form will not be considered.

2 Signature Signature should be in black ink on a white background

Size—3 Kb to 20 Kb

Step 4: Payment of Application Fee

The fees for the application can be paid online via debit card, credit card or net banking, or through E-challan. To pay through E-Challan, candidates will have to download the same and then pay the amount in cash at the nearest State Bank of India only (Please check the banking hours of the nearest branch).

Step 5: Printing the Acknowledgement receipt

Once the candidate has made the payment of the application fee, a confirmation page will be generated. The candidate will also receive a confirmation message on the registered mobile number and a confirmation email on the registered email ID of the candidate.

It is advised to all candidates to take a print copy of the application form and preserve it for further references. Once the confirmation page is generated, it will indicate that the application form of NEET has been successfully submitted.

The documents that should be retained until admission by a candidate are:

  • Printed copy of NEET confirmation page
  • Proof of payment of application fees (online acknowledgment slip or slip provided by State Bank of India after payment)
  • Minimum 6 copies of the photograph that been uploaded by the candidate

With respect to J&K applicants who have opted for seats under 15% All India Quota—a copy of the system generated self-declaration.
